The Endocrine Society recommends morning fasting measurements, while the American Urological Association recommends two early-morning total testosterone measurements obtained on separate occasions before diagnosing testosterone deficiency.

Total testosterone
Total testosterone measures the testosterone circulating in your bloodstream, including testosterone bound to proteins. It is generally the recommended initial test when testosterone deficiency is suspected.
Free testosterone
Free testosterone represents the small fraction that is not bound to proteins and is available to interact with tissues.
Free testosterone is not automatically required for every patient. It becomes particularly useful when:
- Total testosterone is near the lower limit of the reference range
- Symptoms and total testosterone do not appear to match
- A condition that may alter sex hormone-binding globulin is present

Depending on the individual clinical picture, it may include:
Clinical history and symptom assessment

- Erectile dysfunction
- Persistent fatigue or diminished stamina
