Estrogen Management: Why Men Need Estradiol Too

Men need estradiol.

Estradiol plays important roles in libido, erectile function, bone health, mood, cognition, and metabolic function. When men take testosterone, some testosterone naturally converts to estradiol through aromatase, an enzyme found in fat and other tissues.

The goal is not to eliminate estradiol. The goal is balance.

Too little estradiol may contribute to joint discomfort, low libido, mood changes, or bone-health concerns. Too much estradiol in a symptomatic man may contribute to breast tenderness, fluid retention, mood changes, or other concerns.

The Endocrine Society recommends monitoring men on testosterone therapy using a standardized plan that includes symptom response, adverse effects, testosterone levels, hematocrit, and prostate cancer risk assessment during the first year of therapy.

Another important issue is blood pressure. In 2025, the FDA issued class-wide labeling changes for testosterone products after ambulatory blood pressure monitoring studies showed increased blood pressure with testosterone products. At the same time, the FDA also updated cardiovascular-risk language after reviewing newer cardiovascular outcomes data.

The practical message is clear: testosterone therapy can be appropriate for selected men, but it requires careful monitoring.

Cardiovascular Risk: What the Evidence Says Now

For years, testosterone therapy was surrounded by uncertainty about cardiovascular risk.

The TRAVERSE trial, published in The New England Journal of Medicine in 2023, provided important data. In men with hypogonadism and preexisting or high risk of cardiovascular disease, testosterone replacement therapy was noninferior to placebo for major adverse cardiovascular events.

That is reassuring, but it does not mean testosterone therapy is risk-free or appropriate for everyone. Some safety signals and monitoring considerations remain important, including blood pressure, hematocrit, edema, arrhythmia risk in selected patients, and individual cardiovascular history.

The best interpretation is not “testosterone is dangerous” or “testosterone is harmless.”

The best interpretation is more precise: when used for appropriately diagnosed hypogonadism in carefully selected and monitored men, testosterone therapy can be considered within an individualized risk-benefit framework.
